A case management is aimed to help the person and the people who support the affect party. It should be documented in such a way the client should understand and he can review it to build their own strength. In addition the plans gives the individual a chance to build its’ own strength and be an active participants in their support. The case closure is the most import important and effective that is part of a mutual agreement between the individual and the case plan goals. Where the processes are monitored and reviewed is an indication the decision has been made and only mutual understanding is remaining.
